Community involvement is an essential part of the planning and delivery of health and wellbeing services. In Medway, several community interest companies aim to reduce social isolation by identifying additional support that an individual may need and helping people access local services; whether it's a chat and a cup of tea or support getting to a GP appointment. Also, through a grants scheme, community-focused projects are supported to engage local residents in leading healthier and more active lifestyles.

Understanding the needs of the community is also an essential part of delivering efficient, effective and responsive services. The local care workstream of the Kent and Medway Sustainability and Transformation Partnership (STP) aims to address some of the issues that impact on the health and wellbeing of the local population and transform the way in which services are provided. The key focus is to build on and further develop local community assets.
